ABSTRACT

Introducción: La toxoplasmosis es una enfermedad parasitaria, en América Latina la prevalencia es relativamente alta, se sabe que la enfermedad no solo es la causa más frecuente de uveítis posterior, sino también la causa principal de la uveítis en sí misma. En Paraguay no se dispone de datos epidemiológicos publicados sobre la misma. Objetivo: Determinar las características de las lesiones retinianas en pacientes con diagnóstico de Toxoplasmosis ocular. Metodología: El presente estudio es observacional, descriptivo, de corte transversal, temporalmente retrospectivo, con muestreo no probabilístico de casos consecutivos. Se enfocó en pacientes con diagnóstico de Toxoplasmosis que acudieron al servicio de Oftalmología del Hospital de Clínicas, durante el periodo de junio de 2020 a febrero 2022. Resultados: Se observó un 54,3 % de mujeres, una media de 30,62 +/- 12,96 años y un 62,8 % de procedencia urbana. El 88,3 % de los casos fueron unilaterales, la mitad de episodios primarios, el 93,6 % casos agudos, y el 97,9 % tuvo un mecanismo de trasmisión adquirido. La mayoría se presentó como Panuveítis, las localizaciones más frecuentes de las lesiones retinocoroideas fueron el Polo posterior (extramacular) y la periferia. El principal abordaje terapéutico fue el tratamiento con Trimetoprim Sulfametoxazol y corticoides sistémicos, la duración del tratamiento fue de 6,51 +/- 1,52 semanas, y las principales complicaciones fueron la Vitritis residual y la Hipertensión ocular. Discusión: La toxoplasmosis ocular es una causa importante de morbilidad visual. Se hace fundamental la prevención de la infección por este parásito mediante medidas higiénico-sanitarias, así como el diagnóstico y tratamiento precoz de los pacientes infectados, de tal manera a disminuir las complicaciones asociadas a esta patología.


Introduction: Toxoplasmosis is a parasitic disease, in Latin America the prevalence is relatively high, it is known that the disease is not only the most frequent cause of posterior uveitis, but also the main cause of uveitis itself. In Paraguay there are no published epidemiological data on it. Objective: To determine the characteristics of retinal lesions in patients diagnosed with ocular Toxoplasmosis. Methods: This study is observational, descriptive, cross-sectional, temporally retrospective, with non-probabilistic sampling of consecutive cases. It focused on patients diagnosed with Toxoplasmosis who attended the Ophthalmology service of the Hospital de Clínicas, during the period from June 2020 to February 2022. Results: 54.3% of women were observed, an average of 30.62 +/ - 12.96 years old and 62.8% of urban origin. 88.3% of the cases were unilateral, half were primary episodes, 93.6% acute cases, and 97.9% had an acquired transmission mechanism. Most presented as panuveitis, the most frequent locations of retinochoroidal lesions were the posterior pole (extramacular) and the periphery. The main therapeutic approach was treatment with trimethoprim sulfamethoxazole and systemic corticosteroids, the duration of treatment was 6.51 +/- 1.52 weeks, and the main complications were residual vitritis and ocular hypertension. Discussion: Ocular toxoplasmosis is an important cause of visual morbidity. It is essential to prevent infection by this parasite through hygienic-sanitary measures, as well as early diagnosis and treatment of infected patients, in order to reduce the complications associated with this pathology.

INTRODUCCIÓN: La toxoplasmosis ocular (TO) es una retinocoroiditis que evoluciona con varios episodios de inflamación y puede presentarse, tanto en la forma congénita o adquirida de la enfermedad, OBJETIVO: Describir la frecuencia y características clínicas de la TO en lactantes de 0 a 12 meses, hijos de madres con serología positiva para toxoplasmosis en el periodo perinatal. METODOLOGÍA: Estudio descriptivo transversal, ambispectivo. Ingresaron lactantes de 0 a 12 meses de edad, cuyas madres tenían serología positiva para toxoplasmosis en el periodo perinatal, remitidos al servicio de oftalmología pediátrica para evaluación. Se recogieron variables demográficas, serología materna y de los lactantes, y los resultados del examen oftalmológico. Los datos fueron analizados en SPSS-v21. RESULTADOS: El 46,4% de 125 lactantes tenían TO, de ellos, 67,2% era de sexo femenino (p = 0,04), la mediana de edad fue de 6 meses, el 41% tenía IgG e IgM positiva. Las lesiones fueron bilaterales en 82,8%, central en 86,2%, e inactivas en 81%. La retinocoroiditis se acompañó de estrabismo en 41%. CONCLUSIONES: La frecuencia de TO en esta población de lactantes con toxoplasmosis congénita, fue elevada. Más de 80% de las lesiones oculares eran inactivas, de localización central y compromiso bilateral.


BACKGROUND: Ocular toxoplasmosis (OT) is a retinochoroiditis that evolves with several episodes of inflammation and can occur both in the congenital or acquired form of the disease, AIM: To describe the frequency and clinical characteristics of OT in infants aged 0 to 12 months, children of mothers with positive serology for toxoplasmosis in the perinatal period. METHODS: Cross-sectional descriptive, ambispective study. RESULTS: Infants from 0 to 12 months of age, whose mothers had positive serology for toxoplasmosis in the perinatal period, referred to the pediatric ophthalmology service for evaluation, were admitted. Demographic variables, maternal and infant serology and the results of the ophthalmological examination were collected. Data were analyzed in SPSS v21 RESULTS: 46.4% of 125 infants had OT, of them 67.2% were female, (p = 0.04) the median age was 6 months, 41% had IgG and IgM positive. The lesions were bilateral in 82.8%, central in 86.2%, and inactive in 81%. Retinochoroiditis was accompanied by strabismus in 41%. CONCLUSIONS: The frequency of OT in this population of infants with congenital toxoplasmosis was high. more than 80% of the eye lesions were inactive, centrally located and bilaterally involved.

ABSTRACT

Objetivo: Determinar la evolución del resultado visual en pacientes con toxoplasmosis ocular activa. Métodos: Se realizó un estudio observacional prospectivo longitudinal en 101 pacientes inmunocompetentes con toxoplasmosis ocular activa, atendidos en la consulta de Uveítis del Hospital General Docente "Abel Santamaría", desde enero de 2012 a diciembre de 2018. Se evaluaron las variables localización de la lesión, tamaño, número, episodio, grado de inflamación, complicaciones, recurrencia postratamiento y mejor agudeza visual corregida. Se analizaron los resultados utilizando frecuencias absolutas y relativas, la asociación estadística chi cuadrado, las pruebas U Mann-Whitney o Kruskall Wallis, Friedman y de rangos con signos de Wilcoxon. Resultados: Según la localización de la lesión, los resultados visuales inferiores se presentaron en los pacientes con lesiones en zona I y los mejores se obtuvieron cuando hubo afectación en zona III. Se mostró una mejor evolución del resultado visual en los que tuvieron lesiones menores o iguales a un diámetro papilar. Existió diferencia estadística entre los diferentes grados de gravedad de la inflamación, con tendencia al incremento de la mejor agudeza visual corregida en el tiempo, después del tratamiento. Conclusiones: Durante la evolución de los pacientes inmunocompetentes con toxoplasmosis ocular activa se logra mejoría de la visión(AU)


Objective: Determine the evolution of visual results in patients with active ocular toxoplasmosis. Methods: An observational longitudinal prospective study was conducted of 101 immunocompetent patients with active ocular toxoplasmosis attending the Uveitis Service at Abel Santamaría General University Hospital from January 2012 to December 2018. The variables evaluated were injury location, size, number, episode, degree of inflammation, complications, post-treatment recurrence and best corrected visual acuity. Results were analyzed with absolute and relative frequencies, chi-square statistical association, the Mann-Whitney U or Kruskall Wallis tests, the Friedman test and the Wilcoxon signed-rank test. Results: According to injury location, the lowest visual results were obtained in patients with zone I lesions, whereas the best results corresponded to zone III lesions. A better visual result evolution was achieved in patients with lesions smaller than or equal to a papillary diameter. A statistical difference was found between the various degrees of inflammation severity, with a tendency to an increase in best corrected visual acuity with the passing of time after treatment. Conclusions: Visual improvement is achieved during the evolution of immunocompetent patients with active ocular toxoplasmosis(AU)

Introdução: A toxoplasmose é uma infecção de distribuição cosmopolita, tendo como um dos principais achados a toxoplasmose ocular. Contudo, existem divergências quanto à eficácia dos tratamentos atualmente propostos. Objetivos: Revisar na literatura científica os tratamentos recomendados para retinocoroidite/corioretinite toxoplásmica. Métodos: Foi realizada uma busca de artigos publicados nos bancos de dados MEDLINE e LILACS, utilizando os descritores "retinocoroidite", "toxoplasmose" e "tratamento", assim como as traduções para o inglês. Foram selecionados trabalhos publicados no período de 2010 a março de 2020. Obtiveram-se 612 resultados na busca, sendo 7 artigos elegíveis para avaliação integral. Os parâmetros analisados foram a melhora na acuidade visual corrigida, número de recorrências e efeitos adversos. Resultados: Todos os esquemas terapêuticos apresentaram melhora no valor da melhor acuidade visual corrigida, mas não foi evidenciado superioridade ao esquema gold standard ou ao placebo. A Trimetoprima e Sulfametoxazol apresentaram grande eficácia em evitar recorrência das lesões. O esquema tradicional (Pirimetamina, Sulfadiazina e Prednisolona) foi o único que apresentou efeitos adversos graves. Discussão: Nesta revisão, não foram encontradas evidências de que os tratamentos alternativos analisados são superiores ao esquema tradicional ou ao placebo quanto à melhora da acuidade visual. A Trimetoprima e Sulfametoxazol orais foram eficazes em evitar recorrência de novas lesões em um período de 3 anos. Recomenda-se sejam realizados novos ensaios clínicos randomizados com acompanhamento a longo prazo visando confirmar estas questões.


Introduction: Toxoplasmosis is a cosmopolitan infection, and one of the main findings is the ocular toxoplasmosis. However, there are disagreements regarding the effectiveness of the proposed treatments. Objectives: The aim of this article is to review the recommended treatments for toxoplasmic retinochoroiditis/chorioretinitis in the scientific literature. Methods: It was performed a search for articles published in the PubMed and LILACS databases, using the descriptors "retinochoroiditis", "toxoplasmosis" and "treatment", as well as the translation into Portuguese. There were included works published in the period from 2010 to March 2020. The initial research found 612 papers, and 7 articles were eligible for full evaluation. The parameters analyzed were the best corrected visual acuity (BCVA), number of recurrences and adverse effects. Results: All therapeutic regimens showed an improvement in BCVA value, but there was no evidence of superiority to the gold standard or placebo regimen. Trimethoprim and Sulfamethoxazole were highly effective in preventing recurrence of lesions. The traditional regimen (Pyrimethamine, Sulfadiazine and Prednisolone) was the only one that had serious adverse effects. Conclusion: In this review, no evidence was found that the alternative treatments analyzed are superior to the traditional regimen or to placebo in terms of improving visual acuity. Oral Trimethoprim and Sulfamethoxazole were effective in preventing recurrence of new lesions over a 3-year period. It is recommended that new randomized controlled trials with long-term follow-up be performed to confirm these issues.

ABSTRACT

Ocular toxoplasmosis is caused by Toxoplasma gondii, inducing retinochoroiditis. It is the leading cause of infectiousposterior uveitis worldwide. Its treatment is based on oral drug administration. However, the blood–ocular barriersystems make the penetration of therapeutic drug concentrations within the eye difficult, limiting the effectiveness oftreatments. In this context, ocular drug delivery systems represent therapeutic alternative for the treatment of oculartoxoplasmosis. In this study, a review of clinical manifestations, diagnosis, treatment, and perspectives regardingthe treatment of ocular toxoplasmosis was conducted. A search was carried out on ScienceDirect, Scopus, Webof Science, PubMed, and SciELO, and the following keywords were used: toxoplasmosis, ocular toxoplasmosis,toxoplasmic retinochoroiditis, and congenital toxoplasmosis; and Boolean operators, associated with other keywords,such as epidemiology, ocular toxoplasmosis diagnosis, ocular toxoplasmosis treatment, and ocular toxoplasmosisperspectives, were applied. In conclusion, ocular toxoplasmosis still lacks effective treatment. Therefore, it is essentialto develop new molecules and/or new drug delivery systems capable of releasing therapeutic doses of anti-Toxoplasmadrugs directly in the posterior segment of the eye, for an extended period, since complications resulting from thedisease may shorten the productive life of individuals and may even lead to blindness

ABSTRACT Purpose: To evaluate the prevalence, clinical characteristics, and types of optic nerve involvement in patients with ocular toxoplasmosis. Methods: For this retrospective cross-sectional study, we examined all patients with active ocular toxoplasmosis referred to our Uveitis Section during the last 12 years, and we included patients with optic nerve involvement in the study. The primary outcome was the prevalence of optic nerve involvement, and secondary outcomes included the types of optic nerve involvement and the final best-corrected visual acuity after treatment. Results: The prevalence of optic nerve involvement was 14.4%, with the leading cause being the activation of a juxtapapillary lesion (70.5%). We found papillitis in two eyes and neuroretinitis in two eyes (11.7% for each). We only detected one optic nerve involvement secondary to a distant active lesion (5.8%). Sixteen patients (94.1%) had unilateral ocular toxoplasmosis. The overall final best-corrected visual acuity after treatment was 10/10 (LogMAR = 0.0) excluding the three patients with a juxtapapillary scar involving the macula. Conclusions: Optic nerve involvement was common in patients with ocular toxoplasmosis. The main type of optic nerve involvement was caused by activation of an old juxtapapillary lesion. Treatment was quickly effective, but the best-corrected visual acuity was dependent on the presence of a scar in the papillomacular bundle.


RESUMO Objetivos: Avaliar a prevalência, características clínicas e tipos de acometimento do nervo óptico em pacientes com toxoplasmose ocular. Métodos: Para este estudo retrospectivo transversal, examinamos todos os pacientes com toxoplasmose ocular ativa encaminhados ao nosso Setor de Uveíte nos últimos 12 anos, e incluímos pacientes com comprometimento do nervo óptico no estudo. O resultado primário foi a prevalência do envolvimento do nervo óptico, e os resultados secundários incluíram os tipos de envolvimento do nervo óptico e a acuidade visual final melhor corrigida após o tratamento. Resultados: A prevalência de acometimento do nervo óptico foi 14,4%, sendo a principal causa a ativação de uma lesão justapapilar (70,5%). Encontramos papilite em dois olhos e neuroretinite em dois olhos (11,7% para cada um). Apenas detectamos um comprometimento do nervo óptico secundário a uma lesão ativa distante (5,8%). Dezesseis pacientes (94,1%) apresentavam toxoplasmose ocular unilateral. A acuidade visual final com melhor correção após o tratamento foi 10/10 (LogMAR= 0,0) excluindo os três pacientes com uma cicatriz justapapilar envolvendo a mácula. Conclusões: O comprometimento do nervo óptico foi comum em pacientes com toxoplasmose ocular. O principal tipo de comprometimento do nervo óptico foi causado pela ativação de uma lesão justapapilar antiga. O tratamento foi rapidamente eficaz, mas a acuidade visual final com melhor correção foi dependente da presença de uma cicatriz no feixe papilomacular.

PURPOSE: This study sought to describe the different clinical features and presentations of primary ocular toxoplasmosis in a setting not demonstrating an outbreak of disease.METHODS: This was a retrospective review of patients presenting to uveitis management services in Auckland and Hamilton, New Zealand between 2003 to 2018 with uveitis and positive toxoplasmosis immunoglobulin M serology.RESULTS: We identified 16 patients with primary acquired toxoplasmosis infection and ocular involvement. The mean age was 53 years. Systemic symptoms were reported in 56% (9 / 16). Visual acuity was reduced to 20 / 30 or less in 50% of patients (8 / 16). A single focus of retinitis without a pigmented scar was the salient clinical feature in 69% (11 / 16). Optic nerve inflammation was the sole clinical finding in 19% (3 / 16). Bilateral arterial vasculitis was the sole clinical finding in 13% (2 / 16). A delay in the diagnosis of toxoplasmosis of more than two weeks occurred in 38% (6 / 16) due to an initial alternative diagnosis. Antibiotic therapy was prescribed in all cases. Vision was maintained or improved in 69% (11 / 16) at the most recent follow-up visit (15 months to 10 years). Relapse occurred in 69% (11 / 16), typically within four years from the initial presentation.CONCLUSIONS: Primary ocular toxoplasmosis presenting in adulthood is a relatively uncommon cause of posterior uveitis in New Zealand. This condition should be considered in any patient presenting with retinitis or optic nerve inflammation without a retinochoroidal scar. This disease tends to relapse; thus, close follow-up is required.

Introducción: La toxoplasmosis afecta frecuentemente la cámara posterior del ojo y puede producir pérdida de la visión. Objetivo: Describir la frecuencia y características clínicas de la Toxoplasmosis ocular en un consultorio de oftalmología pediátrica. Materiales y Métodos: Estudio observacional, descriptivo retrospectivo Se analizaron las historias clínicas de pacientes de 0 a 18 años de edad, con toxoplasmosis ocular, atendidos en el consultorio de oftalmología pediátrica, desde enero 2011 a mayo de 2014. Variables: edad, sexo, forma de toxoplasmosis, tipo de lesión ocular, localización de la lesión, compromiso unilateral o bilateral, y complicaciones oculares. Los datos se analizaron con SPSSv21 utilizando estadísticas descriptivas e inferenciales. No fue necesario el consentimiento informado porque se trata de un estudio retrospectivo. Resultados: Ingresaron 72 pacientes. La mediana de edad 48 meses, fueron neonatos el 6,9%. El 52,8% eran sexo femenino. El 80,5% presentaron toxoplasmosis adquirida y el 19, 4% congénita. Todos los pacientes tenían coriorretinitis. Y en el 7% se acompañaron de uveítis. La afectación fue bilateral en 52.8%. Las lesiones eran inactivas en 87,5 con localización central. Presentaron complicaciones en el 58,3% de los pacientes: estrabismo en 76% y en 24 % desprendimiento de retina. Conclusión: La frecuencia de TO fue del 0,8%. La mayoría de los pacientes tenían más de 2 años de edad y el 19% tenía toxoplasmosis congénita. Todos tuvieron corioretinitis, con compromiso bilateral y localización central más frecuentemente. Hubo predominio de lesiones inactivas tanto en la toxoplasmosis congénita como en la adquirida. Las complicaciones fueron estrabismo y desprendimiento de retina.


Introduction: Toxoplasmosis frequently affects the posterior chamber of the eye and can cause vision loss. Objective: To describe the frequency and clinical characteristics of ocular toxoplasmosis in a pediatric ophthalmology clinic. Materials and Methods: this was an observational, descriptive and retrospective study. We reviewed the clinical histories of patients from 0 to 18 years of age, with ocular toxoplasmosis, treated in the pediatric ophthalmology clinic, from January 2011 to May 2014. Variables were age, sex, form of toxoplasmosis, ocular lesion type, lesion location, unilateral or bilateral involvement, and ocular complications. The data was analyzed with SPSSv21 using descriptive and inferential statistics. Informed consent was not necessary because it was a retrospective study. Results: 72 patients were included. The median age was 48 months; 6.9% were neonates, 52.8% were female. 80.5% had acquired toxoplasmosis and 19.4% congenital. All the patients had chorioretinitis and 7% also had uveitis. Bilateral affectation was present in 52.8%. The lesions were inactive at 87.5% with central location. Complications were present in 58.3% of patients: strabismus in 76% and 24% had retinal detachment. Conclusion: The frequency of OT was 0.8%. The majority of the patients were over 2 years of age and 19% had congenital toxoplasmosis. All had chorioretinitis, with bilateral involvement and central location present more frequently. There was a predominance of inactive lesions in both congenital and acquired toxoplasmosis. The most common complications were strabismus and retinal detachment.

Resumo A toxoplasmose ocular congênita é uma manifestação da infecção pelo parasita Toxoplasma gondii que ocorrer por meio placentário. Essa doença pode provocar importantes sequelas visuais. Este trabalho descreve um estudo de caso que utilizou avaliação psicofísica visual para descrever alterações funcionais decorrentes da toxoplasmose ocular congênita. A avaliação foi realizada em paciente de 30 anos, sexo masculino, residente em São Luís (MA), hígido com histórico da mãe ter apresentado infecção por toxoplasmose no período gestacional. Somente aos 26 anos foi feita uma avaliação médica detalhada que descreveu lesão na fóvea do olho direito. Na avaliação clínica clássica e na avaliação psicofísica visual, o paciente apresentou visão normal para o olho esquerdo. O olho direito apresentou baixa acuidade visual (valor decimal: 0,028), campo visual com escotomas localizados até 20º de ângulo visual e alteração da visão de cor. O estudo fornece informações médicas seguras e relevantes para o diagnóstico de toxoplasmose ocular congênita utilizando testes eficazes, de baixo custo e boa portabilidade, possibilitando uma alternativa de diagnóstico funcional para ser aplicada em locais de difícil acesso pelo interior do Brasil.


Abstract Congenital ocular toxoplasmosis is a manifestation of infection by the parasite Toxoplasma gondii that occurs through the placenta. This disease can lead to important visual sequelae. This paper describes a case report that uses visual psychophysics for the alterations of congenital ocular toxoplasmosis. The evaluation was performed in a 30-year-old man, a resident of São Luís (MA), with a history of his mother have had toxoplasmosis in the gestational period. Only at 26 years-old he did the detailed medical evaluation that described a lesion the fovea of the right eye. In the classic clinical examination and in visual psychophysical evaluation, the patient presented normal vision for the left eye. The right eye had low visual acuity (decimal value: 0.028), visual field with scotomas up to 20º of visual angle and alteration of color vision. The study offers safe and relevant medical information for the diagnosis of congenital ocular toxoplasmosis using a low-cost evaluation and with good portability, allowing a functional diagnostic alternative to be applied in places difficult to access in the interior of Brazil.

ABSTRACT

This prospective study was aimed to detect acute and chronic ocular toxoplasmosis by comparison of anti-Toxoplasma gondii IgM and IgG antibody levels and IgG avidity test. One hundred and seventeen patients with ocular toxoplasmosis (OT) who referred to the Farabi Eye Hospital, Tehran, Iran were included in this study. Of the patients, 77 cases were positive for anti-T. gondii IgG, and 8 cases were positive for anti-T. gondii IgM. IgG avidity test revealed 11, 4, and 102 cases were low, intermediate, and high, respectively, and 6.8% and 9.4% of cases were positive for IgM and IgG avidity tests, respectively (P=0.632). Agreement (Kappa value) between paired tests IgG-IgM, IgG-IgG avidity, and IgM-IgG avidity was 0.080, 0.099, and 0.721, respectively (P < 0.05). This study showed that conventional serologic tests (IgM and IgG levels) and IgG avidity correlate well each other and can be used to differentiate recent infections from old OT. It seems that reactivated old infections rather than recently acquired infections are majority of Iranian OT patients.

Objetivo: Describir aportes investigativos en Toxoplasmosis ocular, realizados en Colombia. Método: Revisión narrativa de la literatura entre 2007 y 2017 en Toxoplasmosis Ocular (TO) en Colombia, sobre aspectos relacionados con el parásito y con el hospedero. Se incluyeron las revisiones publicadas, así como todos los artículos originales relacionados con diversos temas: epidemiológicos, inmunológicos, clínicos, diagnósticos y terapéuticos a través de PubMed usando los términos MeSH, con la siguiente sintaxis: "(((Colombia[Affiliation]) OR colombia[MeSHTerms]) OR south america[MeSH Terms]) AND "toxoplasmosis, ocular"[MeSH Terms] AND ("2007/01/01"[PDAT] : "2017/12/31"[PDAT]). Adicionalmente se realizaron búsquedas manuales. Resultados: En la búsqueda de la literatura con los términos MESH mencionados, se encontraron 36 artículos, distribuidos en el territorio con 10 en Colombia y 26 en el resto de Suramérica. Se encontraron diferencias importantes relacionadas con el parásito y con el hospedero, al comparar Colombia con el mundo. Las cepas en Colombia son más virulentas (Tipo I/III y atípicas). Se encontró una alta prevalencia de títulos positivos y desarrollo de TO en el país, presentándose el agua y los productos cárnicos como uno de los principales medios de contagio. Se determinó la importancia de péptidos que activan la respuesta inmune humoral en pacientes. Se encontraron diferencias significativas en la severidad clínica (tamaño de las lesiones, grado de inflamación, compromiso macular, compromiso bilateral y recurrencias, siendo mayores para los pacientes colombianos) relacionada con la respuesta inflamatoria producida en el hospedero, asociada con una disminución de los niveles intraoculares de IFN-γ /IL-17 y un aumento de IL-6/Il-13. En los pacientes colombianos el citoquinoma ocular está relacionado con las características clínicas, presentando una respuestaTh 2 relacionada con un compromiso severo ocular. Conclusión: Los aportes investigativos que Colombia ha realizado en TO, han sido de importancia mundial, permitiendo conocer más acerca del tipo de parásito, la epidemiología, la presentación clínica y la respuesta inmune.


Purpose: To describe research contributions in ocular toxoplasmosis performed in Colombia. Method: We conducted a narrative review of the literature on ocular toxoplasmosis (OT) in Colombia between 2007 and 2017 on aspects related to the parasite and to the host. Published reviews were included, as well as all the original articles related to various topics: epidemiological, immunological, clinical, diagnostic and therapeutic through PubMed using the MeSH terms, with the following syntax: "(((Colombia [Affi liation]) OR colombia [MeSH Terms]) OR south america [MeSH Terms]) AND "toxoplasmosis, ocular" [MeSH Terms] AND ("2007/01/01" [PDAT]: "2017/12/31" [PDAT]). Hand searches were also conducted. Results: In the literature search with the MESH terms, 36 articles were found, distributed in the territory with 10 in Colombia and 26 in the rest of South America. Important diff erences were found in the relationship between the parasite and the host when comparing Colombia with the rest of the world. The strains in Colombia are more virulent (Type I / III and atypical) and we found a high prevalence of positive titers and development of OT in the country, with water and meat products as one of the main means of contagion. The importance of peptides that triggered the humoral immune response in patients was determined. Significant differences were found in the clinical severity (with Colombian patients experiencing a greater size of lesions, degree of infl ammation, macular involvement, bilateral involvement, and recurrences) related to the infl ammatory response produced in the host, associated with a decrease in intraocular levels of IFN-γ / IL-17 and an increase of IL-6 / Il-13. In Colombian patients, ocular cytokinome is related to clinical features, presenting a Th 2 response related to severe ocular involvement. Conclusion: Research contributions of OT in Colombia have been of global importance, allowing to know more about the type of parasite, the epidemiology, the clinical presentation, the immune response.

Introdução: A primo-infecção pelo Toxoplasma gondii na gestação causa um risco de transmissão fetal entre 10% e 100%. Em 2006, a 15ª Regional de Saúde do Paraná criou a Rede de Controle da Toxoplasmose Gestacional e Congênita com referência para o Hospital Universitário de Maringá. Objetivo: avaliar as medidas adotadas pelos profissionais da Atenção Primária, para o controle da toxoplasmose gestacional e ocular. Método: Foram sorteadas 20% das 39 Unidades Básicas de Saúde de quatro municípios pertencentes à 15ª Regional de Saúde. A abordagem aos profissionais que assistiam as gestantes foi mediada por um questionário estruturado. Os dados coletados foram sobre as medidas realizadas: com gestantes soro não reagentes, com suspeita aguda, e com toxoplasmose ocular. Resultados: A maioria dos profissionais desconhecia a rede de controle, maneiras de infecção e medidas profiláticas para toxoplasmose. Conclusão: Isto mostra a importância da atualização profissional e perseverança da educação continuada (AU)


INTRODUCTION The primary infection by Toxoplasma gondii in pregnancy leads to a 10%--100% risk of fetal transmission. In 2006, the15th Health Division of the state of Paraná created the Network for Gestational and Congenital Toxoplasmosis Control with reference to the University Hospital of the city of Maringá. OBJECTIVE to evaluate the measures taken by primary care professionals regarding the control of gestational and ocular toxoplasmosis. METHOD Twenty percent of the 39 Basic Health Units in four municipalities belonging to the 15th Health Division of Paraná. The professionals who attended the pregnant women were interviewed through structured questionnaires. Data were collected on the measures carried out with non-reactive pregnant women, with acute suspicion, and ocular toxoplasmosis. RESULTS Most professionals were unaware of this control network, of the mode of infection, and the toxoplasmosis prophylactic measures. CONCLUSION This shows the importance of professional updating, and perseverance in continuing education (AU)

Este estudo avaliou os níveis de expressão de mRNA de 9 citocinas empacientes com toxoplasmose ocular e disseminada. Foram analisadas 77 amostrasclínicas divididas em 4 Grupos. Foram incluídos no estudo amostras de autopsiasparafinadas de cérebro e pulmão provenientes de 11 pacientes com a coinfecçãotoxoplasmose disseminada e AIDS (Grupo I). As células mononucleares do sangueperiférico (PBMC) foram isoladas de 23 pacientes com toxoplasmose ocular (GrupoII) e 9 indivíduos com toxoplasmose crônica) (Grupo III). A seguir, as amostras dePBMC foram estimuladas in vitro com antígeno de Toxoplasma gondii. Os controlesnegativos (Grupo IV) foram constituídos por 7 amostras de PBMC de indivíduossoronegativos para toxoplasmose e 16 amostras de tecido parafinado (cérebro epulmão) de pacientes negativos para toxoplasmose e HIV. A expressão de mRNApara IFN-γ, TNF-α, TGF-β, IL-4, IL-6, IL-10, IL-12, IL-17 e, IL-27 foi determinadapor qPCR. Os resultados mostraram um aumento significativo dos níveis deexpressão de mRNA de IL-6 e IL-10 nos pacientes com toxoplasmose ocular.Esses pacientes também apresentaram menor expressão de mRNA de IFN-γ doque os indivíduos crônicos. Nos pacientes com a coinfecção toxoplasmosedisseminada e AIDS foram detectados níveis de expressão de mRNA de TGF-β eIL-6 no cérebro e pulmão. IL-6 foi significativamente menor do que o TGF-β. Até omomento, este é o primeiro estudo a investigar a resposta imune de pacientes comtoxoplasmose sintomática através do perfil de expressão de mRNA das citocinasde interesse.


This study evaluated levels of mRNA expression of 9 cytokines from patientswith ocular and disseminated toxoplasmosis. A total of 77 clinical samples wasanalyzed and they were divided into 4 Groups. The Group I was constituted ofautopsy samples (cerebral and pulmonary paraffin tissues) from 11 patients withdisseminated toxoplasmosis and AIDS. The peripheral blood mononuclear cells(PBMC) were isolated from 23 patients with ocular toxoplasmosis (Group II) and 9individuals with chronic toxoplasmosis (Group III). Next, PBMC samples werestimulated with in vitro with a crude Toxoplasma gondii antigen. Negative controls(Group IV) consisted of 7 PBMC from patients seronegative for toxoplasmosis and16 paraffin tissue samples (brain and lung) from patients negative for toxoplasmosisand HIV. mRNA expression for IFN-γ, TNF-α, TGF-β, IL-4, IL-6, IL-10, IL-12, IL-17and IL-27 was determined by qPCR. The results showed a significant increase inlevels of mRNA expression for IL-6, IL-10 and TGB-β in patients with oculartoxoplasmosis. These patients also had lower mRNA expression for IFN-γ thanthose chronic individuals. In patients with disseminated toxoplasmosis and AIDS,levels of mRNA expression for TGF-β and IL-6 were detected in the brain and lung.IL-6 was significantly lower than TGF-β. To date, this is the first study to investigatean immune response of patients with symptomatic toxoplasmosis through themRNA expression profile of the cytokines of interest.

ABSTRACT

ABSTRACT A 22-year-old man complained of low visual acuity and pain in his left eye for five days. His ophthalmological examination revealed 2+ anterior chamber reaction and a white, poorly defined retinal lesion at the proximal portion of the inferotemporal vascular arcade. There were retinal hemorrhages in the inferotemporal region extending to the retinal periphery. In addition, venous dilation, increased tortuosity, and ischemic retinal whitening along the inferotemporal vascular arcade were also observed. A proper systemic work-up was performed, and the patient was diagnosed with ocular toxoplasmosis. He was treated with an anti-toxoplasma medication, and his condition slowly improved. Inferior macular inner and middle retinal atrophy could be observed on optical coherence tomography as a sequela of ischemic injury. To our knowledge, this is the first report of combined retinal branch vein and artery occlusion in toxoplasmosis resulting in a striking and unusual macular appearance.


RESUMO Um paciente do sexo masculino, com 22 anos de idade, queixou-se de redução da acuidade visual no olho esquerdo por 5 dias. O exame oftalmológico mostrou reação de câmara anterior 2+ e uma lesão retiniana esbranquiçada, pouco definida, na porção proximal da arcada vascular temporal inferior. Foram observadas hemorragias retinianas na região temporal inferior estendendo-se à periferia, assim como ingurgitamento venoso, aumento da tortuosidade e palidez isquêmica da retina no mesmo quadrante. Exames laboratoriais corroboraram o diagnóstico de toxoplasmose ocular. O paciente melhorou lentamente após tratamento apropriado. Foi evidenciada atrofia da retina macular inferior interna e média à tomografia de coerência óptica, como sequela da isquemia retiniana. Para nosso conhecimento, este é o primeiro relato de oclusão retiniana combinada de ramo arterial e venoso em toxoplasmose ocular, levando a um aspecto fundoscópico atípico e peculiar.

ABSTRACT

Systemic infections that are caused by various types of pathogenic organisms can be spread to the eyes as well as to other solid organs. Bacteria, parasites, and viruses can invade the eyes via the bloodstream. Despite advances in the diagnosis and treatment of systemic infections, many patients still suffer from endogenous ocular infections; this is particularly due to an increase in the number of immunosuppressed patients such as those with human immunodeficiency virus infection, those who have had organ transplantations, and those being administered systemic chemotherapeutic and immunomodulating agents, which may increase the chance of ocular involvement. In this review, we clinically evaluated posterior segment manifestations in the eye caused by hematogenous penetration of systemic infections. We focused on the conditions that ophthalmologists encounter most often and that require cooperation with other medical specialists. Posterior segment manifestations and clinical characteristics of cytomegalovirus retinitis, endogenous endophthalmitis, toxoplasmosis, toxocariasis, and ocular syphilis are included in this brief review.

Introduction: Toxoplasmosis is a worldwide disease; it can cause decreased vision or even blindness. The route of transmission in humans may vary according to the habits of the region; probably the ingestion of raw or undercooked meat is the main source of infection. Objective: To determine the seroprevalence of toxoplasmosis in an eye clinic, the frequency of ocular toxoplasmosis (OT) and risk habits for acquiring the infection. Materials and Methods: Adult patients consulting in the Retina Department of the Teaching Hospital of the National University of Asuncion, Paraguay between August and September, 2014 were included. Prior informed consent, socio-demographic and epidemiological data related to T. gondii infection were obtained. In addition a blood sample for the determination of anti T. gondii IgG antibodies by the ELISA method was taken and ophthalmologic evaluation for the diagnosis of OT was made. Results: A total of 80 patients with mean ± SD age of 53 ± 20 years were studied, with slight predominance of women (55%). The seroprevalence of toxoplasmosis was 84% (67/80) and OT was detected in 8.9% of the 67 seropositive persons. The habit of not washing vegetables with sodium hypochlorite and eat meat from wild animals was related to higher risk of infection in this population. Conclusion: It is important to conduct research at the population level to establish the epidemiology of toxoplasmosis in our country. Information on prophylactic measures to prevent infection by T. gondii should be given to the population.


Introducción: La toxoplasmosis es una enfermedad de distribución mundial, que puede ocasionar disminución de la visión hasta ceguera. La vía de transmisión en el hombre puede variar de acuerdo a los hábitos de cada región, siendo probablemente la ingestión de carne cruda o mal cocida la principal vía de contagio. Objetivo: Determinar la seroprevalencia de toxoplasmosis en una clínica oftalmológica, la frecuencia de toxoplasmosis ocular (TO) y los hábitos de riesgo para adquirir la enfermedad. Pacientes y Métodos: Fueron incluidos 80 pacientes adultos que consultaron en el Departamento de Retina de la Cátedra de Oftalmología del Hospital de Clínicas entre agosto y septiembre de 2014. Previo consentimiento informado, se obtuvieron los datos socio-demográficos y epidemiológicos relacionados a la infección por Toxoplasma gondii. Además se tomó una muestra de sangre para la determinación de anticuerpos del tipo IgG anti T. gondii por el método de ELISA y se realizó la evaluación oftalmológica para el diagnóstico de TO. Resultados: La edad promedio ± DE fue de 53 ± 20 años, con leve predominio de mujeres (55%). La seroprevalencia de toxoplasmosis fue de 84% (67/80) y la TO se detectó en 8,9% de los 67 seropositivos. Se observó que el hábito de no lavar las verduras con hipoclorito de sodio y comer carne silvestre presentó mayor riesgo de contraer la infección en esta población. Conclusión: Es importante realizar trabajos de investigación a nivel poblacional para establecer la epidemiología de la toxoplasmosis en nuestro país. Se debe dar a conocer a la población las medidas de profilaxis para evitar la infección por T. gondii.

ABSTRACTPurpose:To evaluate the visual function and architecture of the central and peripapillary retina in patients with inactive toxoplasmic retinochoroiditis outside the macular and peripapillary regions (zones 2 and 3).Methods:Cross-sectional study of 20 eyes (18 patients) with zone 2 and 3 toxoplasmic scars and visual acuity ≥20/25. Patients underwent Humphrey 10-2 perimetry, contrast sensitivity (Mars test), and color vision testing (L'Anthony desaturated D-15). The retinal nerve fiber layer (RNFL) and macular thicknesses were determined by optical coherence tomography.Results:The patients' mean age was 27.4 ± 10.3 years, and the mean duration of remission was 6.15 ± 5.19 months. Abnormal contrast sensitivity and color vision were observed in three (15.0%) and four eyes (20.0%), respectively. Mean deviation (MD) and pattern standard deviation (PSD) fell outside the 95% normal confidence limits of the perimeter's database in 14 (70.0%) and seven eyes (35.0%), respectively. Foveal and mean RNFL thicknesses were within the normal limits in all eyes. Eyes with zone 2 retinochoroiditis had lower foveal sensitivity than eyes with zone 3 lesions (p=0.041). Eyes with a longer duration of remission had a higher MD (r=0.575; p=0.013) and a lower PSD (r=-0.593; p=0.010).Conclusions:Despite normal central and peripapillary retinal architecture, eyes with inactive zone 2 and 3 toxoplasmic retinochoroiditis can present with abnormal color, contrast, and macular perimetric sensitivity. Zone 2 retinochoroiditis was associated with lower foveal sensitivity, and a longer duration of retinochoroiditis remission was associated with better perimetric parameters (MD and PSD).


RESUMOObjetivo:Avaliar a função visual e arquitetura da retina central e peripapilar em pacientes com retinocoroidite toxoplásmica inativa fora da região macular e peripapilar (zonas 2 e 3).Métodos:Estudo transversal de 20 olhos (18 pacientes) com cicatrizes toxoplásmicas nas zonas 2 e 3 com acuidade visual ≥20/25. Os pacientes foram submetidos à perimetria Humphrey 10-2, teste de sensibilidade ao contraste (Teste Mars) e teste de visão de cores (L'Anthony D-15 dessaturado). As espessuras da camada de fibras nervosas da retina (CFNR) e da mácula foram determinadas pela tomografia de coerência óptica.Resultados:A média de idade dos pacientes foi 27,4 ± 10,3 anos, e a duração média da remissão da retinocoroidite foi de 6,15 ± 5,19 meses. Alterações na sensibilidade ao contraste e cores foram observada em, respectivamente, 3 olhos (15,0%) e 4 olhos (20,0%). Os índices perimétricos mean deviation (MD) e pattern standard deviation (PSD) estiveram fora do intervalo de confiança de 95% do perímetro em 14 olhos (70,0%) e 7 olhos (35,0%), respectivamente. A espessura foveal e da CFNR média estiveram dentro do limite da normalidade em todos os olhos. Olhos com retinocoroidite na zona 2 tiveram menor sensibilidade foveal que olhos com lesões na zona 3 (p=0,041). Olhos com remissão de longa duração tiveram um MD mais alto (r=0,575; p=0,013) e um PSD mais baixo (r=-0,593; p=0,010).Conclusão:Apesar da arquitetura normal da retina central e peripapilar, olhos com retinocoroidite inativa nas zonas 2 e 3 podem apresentar anormalidades da visão de cores, sensibilidade ao contras e perimetria macular. A retinocoroidite na zona 2 está associada a uma menor sensibilidade foveal. Longos intervalos de remissão da retinocoroidite estiveram associados a melhores parâmetros perimétricos (MD e PSD).

ABSTRACT

Objetivo: Describir las características clínico epidemiológicas de la Toxoplasmosis ocular activa, enpacientes del Servicio de Úvea del Instituto Nacional de Oftalmología (INO), desde el 2003 hasta el 2007. Materiales y Métodos: Estudio descriptivo retrospectivo, en el que se utilizaron 1498 historiasclínicas de pacientes del servicio de Úvea del Instituto Nacional de Oftalmología desde enero del 2003hasta octubre del 2007. Se obtuvieron 210 historias con diagnóstico de Toxoplasmosis ocular, siendo excluidos del estudio 109 pacientes por presentar: 16 IgG toxoplasma negativo, 46 sin coriorretinitis exudativa activa, 45 sin IgG toxoplasma y dos con opacidad vítrea intensa. La ceguera legal fue definida como la mejor agudeza visual corregida en el ojo afectado, menor o igual a 20/200. Se utilizaron comovariables: la edad, sexo, lugar de nacimiento y procedencia, características clínicas, tiempo de seguimiento, tiempo de inactivación de la lesión, complicaciones y agudeza visual final. Resultados: De 99 pacientes(102 ojos), el 55,56% fueron del sexo masculino. El grupo etario mayormente comprometido fueel de 21-30 años (31,31%). El 96,97% presentó lesión unilateral. La localización más frecuente fue la perifé-rica (61,90%). La complicación más frecuente: lesiones maculares (23,53%). El 7,07% de pacientes fue VIH(+), sin diferencias en la presentación clínica con respecto a los otros pacientes. Conclusiones: Lafrecuencia de Toxoplasmosis Ocular fue de 14,02%, inferior a los reportes mundiales (22,5%), lapresentación clínica-epidemiológica fue similar a la descrita en la literatura, mayormente en nacidos enel interior del país, en la tercera década de la vida, afección ocular unilateral y localización periférica de lalesión retinal. Objective: To describe clinical - epidemiological characteristics of the ocular active toxoplasmosis, in patients of Uvea's Service of the National Institute of Ophthalmology, from 2003 to 2007. Materials and Methods: Retrospective descriptive study, which used 1498 patient records Uveal Service National Eye Institute from January 2003 to October 2007 as an information source. Were 210 patients obtained with diagnosis of ocular toxoplasmosis. Were 109 patients excluded: 16 IgG toxoplasma negative, 46 without exudative chorioretinitis activate, 45 without IgG toxoplasma, 02 with intense vitreous opacity. Legal blindness was definite to the best visual keenness corrected in the affected, minor eye or like 20/200. Variables were used as age, sex, place of birth and origin, clinical characteristics, time of follow-up, time of unactivation of the injury, complications and visual final keenness. Results: From 99 patients (102 eyes), 55.56 %. The age group mainly damaged was from 21 to 30 years old (31.31 %). 96.97 % showed unilateral injury. The most frequent location: peripheral (61.90 %).The most frequent complication: cataract (8.82 %). 7.07 % were HIV (+), without differences in the clinical presentation regarding the other patients. Conclusions: The predominance of the ocular Toxoplasmosis was 14.02 %, lower than the world reports (22.5 %).The clinical - epidemiologic presentation was similar to that described in the documentation, where the unilateral ocular condition and the peripheral location of the retinal injury affected to those born inside the country (Lima-Callao), in the third decade of the life. ABSTRACT

We report an atypical presentation of Toxoplasma retinochoroiditis with associated scleritis in a young and immunocompetent patient. The diagnosis was done on the basis of Polymerase chain reaction of vitreous sample, and the clinical response to specific treatment. This case highlights the unusual presentation of ocular toxoplasmosis as scleritis.

Introducción: Toxoplasma gondii, agente causal de la toxoplasmosis, es un protozoario intracelular obligado que puede afectar al globo ocular, siendo la causa más común de uveítis posterior. Objetivo: determinar la utilidad de las técnicas serológicas y moleculares para el diagnóstico de la toxoplasmosis ocular en pacientes con uveítis. Métodos: se diseñó un estudio de corte transversal para comparar un grupo de pacientes afectados por coriorretinitis toxoplásmica y otro con signos sugestivos de coriorretinitis no toxoplásmica. En ambos grupos se utilizaron métodos serológicos como la inmunofluorescencia indirecta y la prueba de inmunoensayo enzimático (ELISA), y la reacción en cadena de la polimerasa para el diagnóstico de la toxoplasmosis ocular. Resultados: las técnicas serológicas permitieron detectar los anticuerpos IgG anti-Toxoplasma en 100 % de los pacientes con coriorretinitis toxoplásmica, de ellos fueron positivos a los anticuerpos IgM solo 3 pacientes y 1 tuvo un resultado débil a la avidez IgG. La reacción en cadena de la polimerasa detectó el ADN de Toxoplasma gondii en 15 de los 47 pacientes para una sensibilidad de 31,9 %. Conclusiones: se reporta la detección de anticuerpos anti-Toxoplasma, en todos los casos de coriorretinitis toxoplásmica a través de los métodos serológicos, aunque la técnica más sensible fue la inmunofluorescencia indirecta. Se empleó la detección molecular del ADN de Toxoplasma gondii en pacientes con toxoplasmosis ocular, por primera vez en Cuba. Los resultados permiten sugerir el uso de técnicas serológicas y moleculares que ayuden a confirmar el diagnóstico de infección por Toxoplasma en pacientes con coriorretinitis.


Introduction: Toxoplasma gondii, the causative agent of toxoplasmosis, is an intracellular protozoan that can affect the eye, and be the most common cause of posterior uveitis. Objective: to determine the usefulness of serological and molecular techniques for the diagnosis of ocular toxoplasmosis in patients with uveitis. Methods: a cross-sectional study was designed in order to compare a group of patients with toxoplasmic retinochoroiditis and another affected by non-toxoplasmic retinochoroiditis. In both groups serological methods such as indirect immunofluorescence assay, immunoassay (ELISA), and the polymerase chain reaction were used for the diagnosis of ocular toxoplasmosis. Results: serological techniques allowed detecting anti-Toxoplasma IgG antibodies in 100 % of patients with toxoplasmic retinochoroiditis, and only 3 patients were positive to IgM antibodies. Finally, only one had a weak result for IgG avidity. The polymerase chain reaction detected DNA from Toxoplasma gondii in 15 out of 47 patients for a sensitivity of 31.9 %. Conclusions: this paper reports the detection of anti-Toxoplasma antibodies in all toxoplasmic retinochoroiditis cases by serological methods; but the most sensible method was immunofluorescence assay. The molecular detection of Toxoplasma gondii`s DNA in patients with ocular toxoplasmosis was employed for the first time in Cuba. The results suggest that serological and molecular techniques can be applied to confirm the diagnosis of Toxoplasma infection in patients with toxoplasmic retinochoroiditis.
